About the Diabetes Program

​Welcome to Toronto Rehab’s Diabetes, Exercise, and Healthy Lifestyle Program. Our diabetes program team is here to help you manage your diabetes or prediabetes and reach your exercise and lifestyle goals. This is a group-based, hybrid program meaning that some sessions take place at our centre and you join others virtually (by computer or phone).

Taking part in a program, like this one, can help you manage your blood sugar to prevent or delay health problems that often happen with diabetes. If you have prediabetes, our team can support you to make changes to help prevent diabetes. Already, we have helped thousands of people take control of their health. We are here for you too.

Your virtual diabetes program is 16 weeks long and includes both exercise and education.

Our team will work with you to create an exercise program that is safe and effective for you.

Your education program includes 16 weekly learning plans that you will work through with the support of your diabetes team.

By taking part in your exercise and education program, you will improve your health and gain the knowledge and skills to manage your condition and maintain healthy habits. We look forward to working with you on your journey to better health.

How Do I Join the Program?

Patients who completed our onsite diabetes program increased their physical activity, made healthier food choices, and improved their quality of life.

Ask your doctor to refer you to the Diabetes, Exercise & Healthy Lifestyle Program. Your doctor will need to take the following steps:

  1. Download and complete the Diabetes, Exercise & Healthy Lifestyle Program Referral Form
  2. Gather your medical test results:
    • Current 12-Lead ECG
    • Most recent blood work containing lipids, A1C, blood glucose and microalbumin/creatinine ratio
    • Recent cardiac test results, including stress ECG, MUGA/echo, angiogram, clinic notes, cardiac consult notes
  3. Fax the completed form and medical test results to 416-425-0301

Our team will review the referral form to ensure that it is complete. If any information is missing, we will contact your doctor. A member of our office team will call you with information about your first appointment.
