Hybrid Program Overview

Your hybrid (computer/onsite) program is 16-weeks long and includes:

  • 8 virtual sessions
  • 4 onsite sessions, and
  • 4 self-learning sessions.

Below is the weekly schedule of virtual, onsite, and self-learning sessions. Your Cardiac Rehab Supervisor will send you an education schedule that shows the dates and topics for each of these sessions.

Your program manual describes the weekly focus for clinical care and education in 16 learning plans. The learning plans provide information and tools to help you:

  • Treat your disease
  • Get active
  • Eat healthy
  • Feel well
  • Take control of your health

Most of your learning plans use our video-based education program called THRiVE. THRiVE uses education, goal setting, and action planning to help you make small changes over time to improve your health.
